Most of the body modifications are done with the ABMX plugin, and for the abdomen, they use the pregnant+ plugin.

Now, with the material editor, you can modify many, many things, although they're mostly aesthetic aspects, like nipples, lips, pubic hair, etc.
On the Koikatsu Discord server, they share various things to use and combine, overlays and underlays... I recommend checking it out sometime.

I cannot get the legs and stomach right, do you think you can try to see if you can recreate it? Sorry, I'm really not that good with character creation.
